Summary Specialist Palliative Care Services requires a Psychological Therapist to work as part of their Psychological Team for 15 hours/week. You will ideally have experience of working with patients and their families in a healthcare environment and/or working effectively in a multi-disciplinary team. Ideally, you will also hold another qualification in nursing, allied healthcare or social work. You will deliver evidence based psychological assessment and intervention to patients and relatives in situations where the patient is affected by a life-threatening illness and has complex specialist palliative care needs. This includes working in the context of illness, dying, death and bereavement. We are currently looking for an enthusiastic and efficient Pathway Facilitator where you will be the main point of contact for the MDT for discharge planning from the specialist palliative care unit. Supporting throughout the patients journey, you will work as part of the MDT and closely with external colleagues such as Continuing healthcare, the CCGs and Social Services. Supporting a case management style of working, you will ensure that all patients who require support to leave hospital are managed and facilitated. You need to have experience in the health and social care settings and have excellent communication skills.
